Should I calculate engagement rate by impressions or followers?

Use impressions to measure actions against how often a post was shown. Use followers to measure actions against the size of the account's follower audience. Keep the denominator consistent when comparing posts.

Can a LinkedIn engagement rate be over 100%?

Yes. Multiple actions can be counted for one person, and total actions can exceed the denominator. The calculator does not cap the result at 100%.

Should clicks be included in LinkedIn engagement rate?

It depends on your reporting definition. Include clicks when your source and comparison reports include them; otherwise leave clicks blank. State the definition alongside the result.
